New losses should be reported directly to OFP via the Submit a Claim
link on this website. Loss notices may come from policyholders, an agent/agency or attorney who represents a
policyholder, mortgage companies, or claimants. Documentation for existing claims can also be submitted to
OFP via the Upload Files link on this website.
Important documentation such as photos, estimates, and damage reports can be attached using these links.
The person submitting the loss will receive instant confirmation that OFP has received the information.

received after business hours and during the weekend are directed to voicemail that is monitored by the Claims
